ALL PACKETS MUST INCLUDE:
- Home Campus Online Confirmation Page filled out and signed.
- Medical History forms filled out and signed - Pre Participation Exam to be completed by MD/DO/NP/PA Must be signed and stamped by examining practitioner.

Your student-athlete is required to have medical insurance, optional coverage can be purchased at the link below.
